Form to PDF AI turns the PDF form you already hand out into a Google Form™, then fills your original PDF for every response.

HOW IT WORKS
1. Upload your PDF or Word form in Google Sheets™ or Google Forms™. AI finds every field, including printed lines, boxes and tick boxes on flat PDFs.
2. Review the questions: rename, retype or remove fields and mark the required ones, then create the Google Form™ in one click. In Google Forms™, the questions are added to the form you have open.
3. Share the form. Each new response fills the original PDF and saves it to a folder in your Google Drive™. In Google Sheets™, the PDF link and status also appear on the response row.

Already have responses? In Google Sheets™, select the rows and choose Fill selected rows. In Google Forms™, choose Fill missing PDFs.

WHY KEEP THE ORIGINAL PDF?
The people who receive it expect that exact form. Form to PDF AI writes answers onto your existing layout instead of making you rebuild it as a document template.

WHICH PDF FORMS WORK?
• Fillable PDFs: answers go into the form's own fields
• Flat PDFs: text lines, boxes, tick boxes and one-letter-per-box fields
• Choice questions: ticks, or a circle around "circle one" options
• Non-Latin answers, such as Chinese names
• Word forms (.docx or .doc): turned into a PDF first, then filled the same way
Rotated pages and skewed scans are not supported yet. Signatures are typed names.

Anyone who still emails a PDF to be printed, filled in and scanned back: HR onboarding, clinics and dental practices, schools and clubs, advisers, property and accounting firms.

PRIVACY
Respondents fill a normal Google Form™ and need no second account. PDFs and answers are processed for each request and are not stored on our servers or used to train AI models. Filled PDFs stay in your own Google Drive™.
