Overview
Google Forms™️ supports dropdown questions, but it has no native way to populate them from a spreadsheet. Managing dropdown options manually across multiple forms is time-consuming and error-prone.

Sheetfacade connects Google Sheets™️ to Google Forms™️ so you can look up data from your spreadsheet directly in your form, build conditional dropdowns that filter based on previous selections, and autofill related fields automatically. Update your sheet once, and every connected form reflects the change. No code, no manual updates.

▌How to set up 
Open Sheetfacade > Click "Connect a sheet" > Pick your Google Sheet™️ via Google Picker > Describe what you want in plain English (e.g. "Look up name, email, and department based on employee ID") > Share the Sheetfacade link with your respondents instead of the standard Google Forms™️ link > Your form is live and pulling data from your sheet.

▌Use cases

Price and product lookup forms 
A sales rep or wholesale customer selects a product; the price matching their account or tier appears automatically in the form. No more sending separate price lists to different customers or manually checking pricing spreadsheets. Useful for wholesale order forms, trade pricing, and internal procurement.

Employee and member autofill 
A respondent types or selects an ID or name; Sheetfacade finds the matching record in your sheet and autofills related fields automatically. If no match is found, the respondent sees an error message. Useful for membership renewals, employee leave requests, student enrollment, and wholesale order forms where respondents should not need to re-enter information that is already in your system.

Voucher and ticket validation 
A respondent types or selects a value such as a voucher code, ticket number, or coupon code; Sheetfacade checks your sheet instantly. If the value exists, the respondent can proceed. If it does not exist, they see an error message. Useful for voucher redemption, event check-in, coupon validation, and any form where only known records should be accepted.

Event and session registration
Populate available workshop tracks, time slots, or room options directly from your sheet. When sessions fill up, availability updates automatically as responses come in. When schedules change, update your sheet and the form reflects it at the next sync. No need to edit the form itself.

Medication and product autocomplete 
A respondent starts typing and sees matching suggestions from your sheet instantly. Selecting a suggestion autofills related fields automatically. Ideal for long lists where a dropdown would be impractical — medications, product catalogs, supplier names, or any reference list with hundreds or thousands of entries. Manage the list centrally in your sheet and every connected form always reflects the latest values.

Sheetfacade uses its own form URL to serve responses. This is what enables live lookups, conditional dropdowns, and autofill that are not possible through the standard Google Forms™️ share link. Your form design, questions, and responses still live in Google Forms™️.

FormFacade has been a life saver for our non-profit organization! We host a large exhibition every two years where floral designers pick a piece of artwork at the Virginia Museum of Fine Arts to interpret with flowers. The designers order flowers to create their design. In the past, this has been done via email, paper, pencil, etc. It was very time consuming for the organizers. We started out looking at Google Forms, but it was too limiting. Then we discovered the Add-in FormFacade for Google Forms. It checked all of our boxes. Totaling the number of stems each exhibitor ordered and not allowing them to order more than a specified number (data validation). Using a lookup function to a Google sheet that had the names of the flowers and their colors and category to show all the flowers in that category in a drop down. Being able to add tables and present the fields for ordering in a simple format. Allowing customization of a logo and footer. Sending a PDF of the order in an email to the exhibitor after they placed their order. Sending an email with PDF of the order to the organizer who had to submit the total number of stems from all exhibitors to our florist. Allowing for a customized report showing the data we wanted to see. One of the coolest features was using layman's language to ask the program what you wanted to happen and AI made it happen! As the program was "working," clever little status messages came up to let you know it was still working. They were absolutely wonderful to work with! Even though there was a large time difference, I knew they were working to answer my questions and make the form the way I wanted. I had a What's App number I could text. I had an email address I could use. We met by Google Meet several times. Vipin was so patient with me as I tried to understand what to do next. I felt extremely supported. They also were very willing to receive feedback and implement requests if possible. Our form went live today, and we are hoping it will be easy for our users. Thank you, FormFacade!
