Open your form in Google Forms™ > click on the add-on icon > click Formfacade - Sheets Lookup > select “Lookup Sheets” > Pick your Google Sheet™ via Google Picker > Describe what you want in plain English (e.g. "Look up name, email, and department based on employee ID") > Share the Formfacade link with your respondents instead of the standard Google Forms™ link > Your form is live and pulling data from your sheet.

Price and product lookup forms 
A sales rep or wholesale customer selects a product; the price matching their account or tier appears automatically in the form.
